Technical challenges of advanced hip osteonecrosis managed using a vascularised fibular graft.

چکیده
Management of hip osteonecrosis in the young is challenging, and femoral head preservation surgery is often preferred. We report the first local case of treating advanced hip osteonecrosis with a vascularised fibular graft in a 26-year-old man who presented with left hip pain after a fall. He was diagnosed with left hip osteonecrosis and underwent core decompression and insertion of a vascularised fibular graft. Postoperative radiographs and computed tomography of the hip were performed to assess viability of the graft. In this report, we discuss the technical considerations when operating on Asian patients and how technical difficulties are circumvented. We also report the postoperative outcome and discuss the various methods for monitoring viability of the graft.

Free vascularised fibular graft for post-traumatic osteonecrosis of the femoral head in teenage patients.
Free vascularised fibular grafting has been reported to be successful for adult patients with osteonecrosis of the femoral head (ONFH). However, its benefit in teenage patients with post-traumatic ONFH has not been determined. We evaluated the effectiveness of free vascularised fibular grafting in the treatment of this condition in children and adolescents. We retrospectively analysed 28 hips i...
متن کاملVascularised fibular graft in the management of femoral head osteonecrosis: twenty years later.
The management of osteonecrosis of the femoral head ranges from symptomatic therapy to total hip replacement. Conservative treatment is effective only in small, early-stage lesions. Free vascularised fibular grafting has provided more consistently successful results than any other joint-preserving method. It supports the collapsing subchondral plate by primary callus formation, reduces intra-os...
متن کاملReconstruction and limb salvage using a free vascularised fibular graft for periacetabular malignant bone tumours.
We treated four patients with periacetabular malignant tumours by pelvic reconstruction with a free vascularised fibular graft after resection of the tumour. The mean follow-up period was 32 months (9 to 39). The diagnosis was chondrosarcoma in three patients and osteosarcoma in one. In two patients total resection of the hemipelvis was required and in the other two less, but still massive, res...
متن کاملFree non-vascularised fibular graft for treatment of post-traumatic bone defects.
Post-traumatic bone defects usually occur from severe high-velocity injuries due to road traffic accidents; they may be difficult to fill. We have managed defects of long bones by the use of free non-vascularised fibular grafts harvested subperiosteally and held by screw fixation of the fibular strut ends to the ends of the bone defect, combined with an external fixator in 8 tibial defects and ...
متن کاملResidual nonunion following vascularised fibular graft treatment for congenital pseudarthrosis of the tibia: a report of two cases.
Resection and reconstruction using a vascularised fibular graft is a viable alternative treatment for congenital pseudarthrosis of the tibia, although distal junctional nonunion and residual deformity are known complications that are difficult to treat. We illustrate 2 cases in which bony union was achieved following a technique using fibular grafting and intramedullary nailing, without additio...
